Clearance Forms & Waivers
Required Clearances
Pennsylvania Child Abuse Law requires all prospective Camp Susquehanna Volunteers to obtain the following clearances:
- Report of Criminal Background/History from the Pennsylvania State Police (current within in 60 months of camp).
- A Child Abuse History Clearance (current within 60 months of camp)
- A FBI Fingerprint clearance **Only for those prospective volunteers who have not lived continuously in PA for the past 10 years.
How do I obtain my clearances?
The Child Abuse, Criminal History and FBI clearances can all be applied and paid for electronically with a credit card. The FBI clearance also requires a fingerprint submission. All clearances MUST be submitted to Camp Susquehanna prior to May 1, 2016 in order to be eligible to volunteer.
Criminal Background/History:

- Click on link from CS website
- Click “Submit a New Record Check” – you need to have a credit card handy
- Scroll to the bottom of the screen to accept terms and conditions
- Enter Personal Information so clearance will be mailed to you, click next
- Enter information for record check, click “Enter this Request”
- If you are only entering for yourself, click “Finish” at the bottom of the screen
*If you are entering for multiple people, enter each person’s information separately and click “Enter this Request” for each one - Double check your info and click submit
- Enter payment information and click submit
- After a minute or so, your results will be displayed – print this page for your records AND…

Child Abuse Clearance
Click on this link CHILD ABUSE CLEARANCE
- To create an account, click “Create an Account” then “Next”, then enter personal information
- Click Finish to create the account
- Check your email for temporary password.
- Click on this website again
- Click “Access my Clearances”
- Scroll down and click continue after reading instructions
- Sign in using the password in your email
- Change your password, click “Submit”
- Click “Close window” and resign into your account using your new password.
- Accept Terms and Conditions and click “next”
- Click “Continue”
- At the top right of the page, click “Create Clearance Application”
- Click “Begin” at bottom of page and fill out application – make sure to select to receive a paper copy of the application
- After entering payment information, click “Finalize and Submit Application”
- You will receive an email confirmation
- Once you receive your clearance by mail, make a copy of it and

FBI Fingerprinting Clearance
*This clearance is a multistep process. Click Here: FBI FINGERPRINTING CLEARANCE
- Enter service code 1KG6ZJ (Volunteer)
- Follow on screen directions for registration and payment
- You will then need to visit a fingerprint location with proper Identification (see here for accepted ID documents)
- Once you have received your clearance from the FBI check, you can

Who pays for these clearances?
Prospective volunteers are responsible to pay for both their Criminal History/Background Check and their Child Abuse Clearance. *Volunteers who have not lived in Pennsylvania continuously for the past 10 years are required to obtain a FBI Fingerprinting Clearance WHICH CAMP SUSQUEHANNA WILL REIMBURSE. Once you submit your FBI Clearance to Camp Susquehanna, the leadership team will explain the reimbursement process.

- FBI Fingerprinting Clearance is $27.50 (reimbursable)
